Descriptions of selected treatments

Root Canal Treatment

Sometimes the tooth starts to hurt and it may turn out that the tooth tissues have been destroyed by carious bacteria so seriously that root canal treatment is necessary. What does this treatment look like?

At the beginning of treatment, we give the patient anesthesia. The treated tooth is separated from the rest using a special clamp and rubber dam. This protects the patient against entering into his oral cavity any substances used during the procedure. In addition, the cofferdam ensures sterility of the treatment area.

After thorough cleaning and processing of canals, we use Nanocare Plus, which contains silver and gold nanoparticles. It allows to effectively eliminate bacteria that cause inflammation. The dentist then tightly fills the canals with a biocompatible gutta-percha. It is a substance of natural origin, which is a biocompatible material, which is well tolerated by the body. Gutaperka is introduced into the root canals together with the sealer. The whole treatment is carried out under the control of digitally processed X-ray images, which enable rapid visualization on the computer screen and a very accurate evaluation of the treatment.

Why does the tooth require revision of root canal treatment?

During root canal treatment, as with any medical procedure, complications may occur.
The most common causes of failures are:
– narrow or curved root canals,
– additional root canals that have not been found during the original root canal treatment,
– a broken endodontic file in the root canal of the tooth,
– perforation of the root or the tooth chamber,
– deep secondary decay,
– transferring the infection from other neighboring roots.

Sometimes these treatments also require teeth, in which root canal treatment has been performed long ago. Due to the lack of appropriate tools and devices, years ago there was simply no way to do it that properly.
Years ago, modern microscopes were not common and there were no such precise tools that would be able to reach the end of narrow or curved root canals.

What does the revision of root canal treatment look like?
First of all, such treatment should be carried out at magnification, in a microscope or in surgical loupes.

Secondary root canal treatment is similar to the first one, but it is more complicated and requires detailed diagnostics. Before planning and carrying out the procedure, we perform x-ray imaging and, if required alsco CT-scan of the treated tooth. After thoroughly analysis, the doctor opens the tooth again. Then using the microscope he removes the old filling from the root canal. The canal is thoroughly cleaned, developed, disinfected and drained. The gutta-percha together with the sealer is introduced into the root canal, prepared in this way, as in the original root canal treatment. After the treatment, the tooth is rebuilt using a composite filling or a prosthetic crown.

The apicectomy

In some cases, revision of the root canal treatment can not be performed. An alternative is the microresection of the tooth root. In this procedure, the tooth root tip must first be exposed in order to then be removed together with the inflamed area around the root.
The end of the root canal is closed with MTA cement (retrograde root filling).
The procedure is performed under local anesthesia and with the aid of a surgical microscope.
Sometimes this procedure is recommended along with a revision of the root canal treatment.

Implant consultation

Thanks to the analysis of the possibilities and indications for implantation we are able to present to the patient an initial treatment plan. This plan is based on a clinical examination as also on a pantomographic or CBCT examination, which we perform in the X-ray laboratory in our office.
Sometimes it is advisable to first create diagnostic models.
After completion of the planning, we arrange the appointment for the insertion of the implant.

Implantation

Implantation is the insertion of the implant into the upper or lower jaw. The procedure is performed under local anesthesia.
Depending on the area in which the procedure was performed, the anatomical conditions and the relationship with whether the implantation was performed immediately after tooth extraction, a temporary restoration can be performed to maintain the aesthetics and function.
It is fixed or removable dentures. After about a week, we invite the patient to remove the sutures.

Implant crown

Single prosthetic crowns are the most common dentures on implants.
We have two types of crowns: They are glued to the abutment or screwed to the implant.
To reflect the position of the implant, an impression should be taken with transfers.
An anatomical imprint of the opposing jaw should also be made.
During this visit, the selection of the color tone for the final crown is also chosen.
The crown is made in the dental laboratory.
During the next visit, the dentist can place the final crown on the implant in the oral cavity of the patient.

Exposing the implant

After a certain healing time of the implants, depending on the location of the implants and the extent of the procedure, the implant is finally exposed and a healing screw is applied for a period of about 2 weeks. This should form an optimal aesthetics of the adjacent gums.
Exposure of the implant is performed under local anesthesia. The period is 2 to 6 months.
